Information we collect

We may collect information you provide, including your name, email address, telephone number, state, ZIP code, coverage interests, preferred communication method, appointment information, consent choices, and messages. The website contact and consent process is not intended to collect Social Security numbers, Medicare identification numbers, financial account information, medical records, or detailed health histories. Please do not submit those items through a general website form or ordinary email.

When enrollment assistance begins, additional information may be required by a government Marketplace, insurance carrier, or authorized enrollment system. That information should be entered only through the applicable secure and authorized process.

How we use information

• Respond to inquiries and schedule requested conversations.
• Document permission to contact you and the communication methods you select.
• Understand the general coverage subject you want to discuss.
• Provide insurance education, comparisons, application assistance, and ongoing support.
• Comply with licensing, carrier, Marketplace, Medicare, recordkeeping, fraud-prevention, and other legal obligations.
• Protect our website, systems, users, and business operations.

Microsoft systems and carriers

Website and business information may be stored and processed using authorized Microsoft services, including Power Pages, Dataverse, Bookings, Outlook, and related Microsoft business systems. During an application or enrollment, information may be entered into or transmitted through an insurance carrier, government Marketplace, enrollment platform, or other authorized system necessary to complete your request.

We do not sell personal information. We disclose it only as described in this policy, at your direction, to provide requested services, to authorized service providers, carriers, and enrollment systems, or when required by law.

Analytics and cookies

We may use Google Analytics or similar measurement tools to understand general website usage. Analytics must not receive form contents, names, email addresses, telephone numbers, coverage details tied to an identifiable person, or health information. Analytics providers may use cookies or similar identifiers and may process device, browser, approximate-location, and interaction data under their own terms. When analytics is activated, appropriate notice and consent controls will be implemented where required.

Health information and HIPAA

HIPAA applies to covered entities and business associates as defined by federal law; it does not automatically apply to every insurance brokerage activity. Where AM LIFE performs a function subject to HIPAA or a valid business-associate agreement, we will follow the applicable contractual and legal requirements. Regardless of HIPAA status, we apply reasonable administrative, technical, and organizational safeguards to personal information and limit collection through the public website.

Security

We use access controls, authenticated Microsoft business systems, role-based permissions, and other reasonable safeguards appropriate to the information handled. No internet transmission or storage system can be guaranteed completely secure. Do not send highly sensitive identifiers or payment information through general website forms or ordinary email.

Retention

General inquiry records are ordinarily retained for up to five years unless a shorter or longer period is appropriate. Records connected to applications, enrollment, consent, carrier service, complaints, or legal obligations are retained for the period required by applicable law, regulation, contract, or legitimate business need. Marketplace consent and application-review documentation may be retained for at least ten years as required by CMS. Records are deleted or securely disposed of when retention is no longer required.

Your choices and requests

You may ask to review or correct information, withdraw permission for future communications, or request deletion where deletion is legally and operationally permitted. Withdrawing contact consent does not invalidate actions already taken and may not permit deletion of records we must retain.
